Abstract

Background & Aim: The aim of this research is to find alternatives to the traditional painful palatine local injection when extracting upper temporary teeth through enhancing the efficiency of EMLA cream during an appropriate period for child response (3 minutes) using drug delivery systems ((permeability enhancer (DMSO), oral patches, microneedle patches)) for palatine injection and use only lidocaine injection from the vestibular side.

Materials and methods: The research sample consisted of 75 children, The sample was divided randomly into five groups for each group of 15 children. To apply the studied material or method for each group as follows:

Group 1: (Conventional local palatine injection (control group)).

Group 2: (EMLA cream only).

Group 3: (Chemical permeability enhancer DMSO with EMLA cream).

Group 4: (Oral patches with EMLA Cream).

Group 5: (Microneedle patches dissolved with EMLA Cream).

Physiological scale were also used to assess pain By measuring the heart rate and oxygene rate with a device (pulse oximeter), At the following stages of work (application - probing - extraction) for each of the studied groups.

Results: The results of the study showed that at the 95% confidence level, there weren't statistically significant differences in the average amount of change in the rate of oxygenation between the groups used, regardless of the stage studied. Also, when extracting, there weren't statistically significant differences in the average amount of change in heart rate between the groups used in the research sample, as for the rest of the studied stages (application and probing), it is noted that the value of the significance level is smaller than the value 0.05.

Conclusion: Conventional palatal injection and extraction are both a satisfactory overall procedure, and by using drug delivery systems we are able to complete the procedure in an efficient and more comfortable way for the child.
