Surgical Management For Adenoid Cystic Carcinoma Of The Carina : A Case Report

Abstract

Background & Aim: Primary tracheal tumors are rare. Squamous cell carcinoma and Adenoid cystic carcinoma are the most common malignant tumors. The management of these tumors when involve carina is a challenge due to the anatomical specificity of the carina. The aim of this report is to focus on our treatment approach at Al-Assad University Hospital in Damascus of such neglected complex and rare case.

Case report: A 45-year-old patient presented to Al-Assad University Hospital with a slow progressive dyspnea started from 3 years with inspiratory stridor started from a year ago. We diagnosed carina tumor through bronchoscopy abstracting  90% of the airway, and as a initial management we opened the airway using LASER , the pathologic exam  showed Adenoid Cystic Carcinoma , and final surgery was performed by resection and reconstruction of carina.

Conclusion: Early diagnosis of malignant tracheal tumors is the most important factor affecting survival and surgical treatment is the treatment of choice. The Carina resection is a rare surgical procedure and this procedure remains an exceptional one and should be performed in experienced centers
