The closure of palatial acquired fistula by the use of dorsal tongue flap

Abstract
Background & Aim: Residual defects of the palatal region following resection of tumors and Gunshot wounds (GSWs) of the maxillofacial region can be quite consider as challenging to reconstitution doctors in healing time, due to the complexe nature ,composite of available donor areas and the physiology action of the intraoral region. Further compounding the situation, are increase the difficulty of succsse closing the palatal fistula.
Objective: To evaluate the effectiveness of the Dorsal Glossal Flap in the reconstruction of complex palatal defects resulting from maxillofacial GSWs and tumor resections.
Material & Methods: 17 cases(11gune shots six maxillofacil tumor). We study the effects of age,gender,fistula distant.and athe truma kind. The follow up lasted for one year.
Results: All the cases of the palatal fistulas were closed successfully, without post-operative. The glossal flap at the recipient site remained healthy with no recurrence of the fistula in any of the patients in the next years follow up period. There was a complete resolution of the problem of nasal regurgitation of orally ingested fluids and food particles
Conclusion: In GSWs and tumor surgery, where composite tissue defects are involved, the tongue provides a reliable tissue source for restoring lost tissue bulk as well as ensuring a permanent closure and sealing off the oronasal fistulas. Its reliability can be further increased by avoiding a common complication in the postoperative period brought on by movements of the tongue, by immobilizing the tongue by tethering it to the maxillary teeth and also, maintaining the patient on Nasogastric feeding for the three weeks postoperative period This helps to ensure a successful and predictable take of the flap at the donor site without any complications .
